Help with pattern

I'm knitting a baby sweater and I've just put a bunch of stitches on a holder for the sleeve. The next instruction is to cast on two stitches - I've never cast on in the middle of knitting. Help?:)

You can use any of the single-strand cast-ons; knitted, cable, or backward loop.
