Yarn ball band PDF

One of the ravelry members on the "thrifty knitters" group made this yarn ball band PDF for those balls of yarn you get from thrift store sweaters. Enjoy! :muah:

can anyone share?

This ball band pdf doesn't seem to available anymore --- can anyone share the template with me? I have lots of recycled yarn that I'd like to get labeled. Any and all help greatly appreciated!:muah:
