Don't feel guilty. If he's not moving around and enjoying himself, it's probably time. Having the vet check him out first is a good idea, because there's always the chance that he's sick from something besides his kidneys, but most of the know. If the vet tells you so too, it might help, because you won't second-guess yourself.
