Basically weave in some yarn after it's completed, or even when it's wearing but before you get a hole.

I've got the later working on the socks I'm wearing now because my heels are wearing out before my replaced toes are.
Seems to be working pretty good.
Petroleum based knitter, removing that nasty oil from the ground one skein at a time.
