Questions re: sample shown on increase/decrease pages

On the blue sample swatch on your instructional page for increases and decreases, how do you make the raised edge? Also, are all the increases and decreases done on the edges and not any in the middle?

The edge only appears raised; it appears that way from the sts being increased and decreased. I think the incs/decs are done 2 sts in from the edge.
