Casting on mid-row

Help! I don't understand how to cast on mid-row. I am a continental knitter (and a fairly new one at that), and I do not get this very basic concept.

Occassionally in a pattern, I will see the instruction to knit along a row, and the cast on several stitches, and then continuing knitting. How do you cast on once you are dealing with live stitches in a work?

Move the R needle to the L hand, use a knit or cable CO (with the first st on the needle instead of a slipknot) then swap the needle back to the R hand and continue knitting.

You can also use a backward loop cast on for this kind of situation. In that case you don't need to switch the needles in your hands, just throw backward loops over the right hand needle using the working yarn. I use that often but prefer the cable or knitted cast on if quite a few stitches are needed.
