Need help with pattern

Pattern is from Debbie Bliss book called Angel and the pattern is 'Big shrug' on page 21
After co 102sts knitting 1st rib row p2,k2,p2
the 2nd rib row p2,k2,p2 here is where I'm lost
Place markers at each end of last row.
Next row cast on 44sts k to end.
Do I cast on 44sts onto the 102 sts? then k to end????? what end?
Next row cast on 44sts p to end total 190sts.
Where do I cast on?
