There should be an explanation how to do the C4B and C4F. Normally, for both of them you would slip 2 sts to the cable needle, hold in back or front, k2 from the left needle, then knit the sts from the cable needle.

Lots of people have the same question. Usually the pattern defines C4B or C4F (or whatever the cable stitch is) at the beginning before the instructions or sometimes at the very end in a table of abbreviations. Good question.
