KP La Mer stole - question

Hi all,

I'm working on the La Mer stole from KP and I'm lost (and I haven't even started yet ). The directions read in part:

Holding a second strand of yarn over the thumb and using Twisted German Cast on, cast on...

They then tell you to repeat the charted pattern a certain number of times before you bind off:

Holding a second strand of yarn, bind off using Jeny’s Surprisingly Stretchy Bind Off

What's got me perplexed is the "holding a second strand of yarn" bit. Does this mean that you hold two strands together throughout the pattern (I'm used to seeing this as just "with two strands held together") or just for the CO & BO rows?

If anyone has any experience with this pattern or with this phrasing and can help me out, I'd greatly appreciate it
