stuck on pattern instructions increasing on rows

the pattern is for sleeves and I have just done 11th rows of stocking stitch

The pattern says increase 1 stitch either end on the 11th and every following 12th row.
Do they mean after increasing on row 11 to start counting from 1 on row 12 without increasing and then increase when I get to 12

or do they mean increase on 11th row then the next row then start counting from 1 again and increase on the 12th row again.

It's the words "and every following" that is throwing me

The rest of the pattern says until there are 46 stitches I currently have 40 stitches after increasing on row 11.

You will increase one stitch on each edge on row 11, and from there count every 12 rows to do your increases, so in other words on rows 23, 35, 47, etc.

If you have 40 stitches after increasing on row 11, and each increase row adds 2 stitches (one on each edge), you will need to increase 6 more stitches which is a total of 3 more increase rows. So by increasing on row 23, 35 and 47, it will bring you to 46 stitches.
