foll alt row
what is meant by following alternate row? does it mean when doing stockinette stitch, if the knit row is first, the purl row is the alternate row?

Every following alternate row means every other row. So if you are asked to increase on a knit row in stockinette, the next knit row is the alternate row. For example, you would inc on knit row1 and then on row3, 5, 7 etc for however many times are called for.

why can't they be more specific and say increase on all the even/odd rows.. less confusion

Because that may not be what the pattern really wants you to do. It's a conventional way of writing a pattern, once you know what it means, it's not difficult.
