I'm so confused. I casted on 48 stitches then row 1 says to do 19 stitches (various ones) and then "work pattern B over 13 stitches.Then p4. Work pattern A over 7 stitches, p5. " Patterns A and B have 16 rows. Do I do all 16 rows?

You work row one of each of the patterns on row one, then row 2 of each of the patterns, etc. When you finish the 16 rows, you start over at row 1.
