When you're increasing on a sleeve, think of it expanding outward.
You can kfb in the first and last stitch for the first increase, and when you get to them on the next row, treat them as if the pattern widened. For example, if you were working a
.kkppkkppkkppkk rib, expanding would give you
pkkppkkppkkppkkp until the next increase.
If it's a more complicated pattern, just work the stitches in stockinette until you have enough to work the pattern stitches.
